Capturing Forms, Releases, Waivers and Signatures

SignNow is an Integration that we see some organizations utilizing to accomplish this. However, what we see many organizations doing, is utilizing a Form Template to enter in the Forms that they need to collect a signature on (Releases, liability waivers, etc.). As you can see on the example below, there are many different releases that this organization has placed in a Form Template.

Here, you can see that the adult liability release is the Form that has just been entered in using a section break type question.

Then, utilizing a single line text type question to ask for the participant to type their name agreeing to the statements above.

Next, you will want to add a Task that this Form would be added to.

To do this, you will navigate to the Opportunity Task Templates and utilize a Form Template. Input the information and requirements that you desire, and attach the Form that you created to the Task.

Lastly, you will want to add this task either to your Task List or to the Opportunity for your participants to complete.
