Creating a Form

There are many questions that can be captured within Forms. This is where you will build your application as well as things such as: waivers, releases, travel information, reference questions, and more! 


To get to Forms at the Organization Level you will go to Templates, Form Templates.

In order to build a new form, you will select Add New Form in the upper right. 


After you have chosen Add New Form, you will be taken to where you will build out the form. Give the form a name (Re-name Form-in top right) and then begin selecting your questions. You have many choices when it comes to Type of questions. 


Single Line Text: This provides a simple way to capture a single line of information (great for name, address, DOB, City, State, or any question where the answer is a single sentence or less).


Paragraph Text: This is a great option for questions that would require more text than a single sentence. A question such as "Describe your journey to faith in Christ" would be an example of a question you might ask that would require a more lengthy response.


Drop Down List: This is a way to ask question and provide a drop-down list of answers that the participant would select from.


Single Selection (Radio Button): Similar to Drop Down List, but rather a drop-down, all answers are visible. Participant can select only one answer.


Multiple Selection (Checkboxes): Similar to Single Selection in appearance, but the participant has the ability to select multiple options. 


Section Break and Page Break: Ways to break-up the form. These are helpful when switching topics in a form (possibly a section/page for Health, another for Personal Information, etc). It also will help participants when it comes to saving information. Rather than having one long page, break the form up into a couple pages (the system saves information at the end of every page-this is nice if participants have to come back to the form or if a connection is lost).


Upload File: This allows you to request the participant upload a file to the form. NOTE-another place you can do this is in the Task area (creating a Task requiring a certain form be uploaded - such as a waiver that you need signed).


Passport Information: This is a pre-built question that captures what you would need for passport information. 


Parent/Guardian Email: A question to ask specifically if you are having minors fill out forms and need to have a way to communicate with the parent/guardian.


As you begin, you will notice a couple things with each question you add.
First, is the ability to Save to Templates button. What this means is that the question is saved in the Templates area (next to Type on the right side) and that the question can be reported on (if you run a report, you can collect the answers captured from that question). We recommend ALL questions be Saved to templates. Continue reading more below on Template Questions.


Second, is the Required button. What this does is require an answer from the participant filling out the form before moving on. 



If you have created the question and saved it to templates, you can now see all the questions in Templates (below). Now, rather than creating the same question again, you will want to pull it over from the Templates (if you create the question again, it will make running reports difficult). Searching template questions is easy and will save you time and energy! 



Once you have created questions, you can easily determine if it is a template question by noting the blue flag (below).



After you have finished creating your Form, you can now put it into a Task or use it as an Application in an Opportunity.

Another wonderful feature of Template Questions is that when participants answer these questions, they are saved to that user's data and replicated when a user applies for another Opportunity. (Meaning, if the question was answered before, those answers will auto-populate for your users).
